In 2000 the use of marijuana for documented medical reasons was legalized by the State of Hawaii. Dr. Greenly believes in and has been an advocate of the medicinal value/benefits of marijuana for certain medical conditions and helps patients who meet the State of Hawaii criteria with the application process.
At least one of the following criteria must be met for the State of Hawaii and Dr. Greenly in order to begin the process of application.
- Malignant neoplasm (cancer)
- Severe nausea
- Positive status for HIV or AIDS
- A chronic or debilitating disease or medical condition or its treatment that produces, for a specific patient, one or more of the following:
a) Cachexia/Wasting Syndrome
b) Severe pain
c) Seizures, including but not limited to seizures caused by Epilepsy
d) Persistent muscle spasms, including but not limited to spasms caused by Multiple Sclerosis or Crohn’s Disease
Dr. Greenly requires ALL prospective patients to submit current medical documentation pertaining to their condition either by mail or they can drop them off to his office. No faxes are accepted. Once Dr. Greenly reviews the documentation, someone from his staff will call the patient and either schedule them or inform them that they do not meet the criteria and cannot be scheduled. Documentation in the form of a physician’s letter that confirms a diagnosis is also acceptable.

Once an appointment has been scheduled and confirmed by the office, ALL patients must register online with the State of Hawaii Department of Health before their appointment at: http://health.hawaii.gov/medicalmarijuana/application-information/
Application cost to the Department of Health is $38.50. This payment must be made online by debit or credit card to complete your application.
If a patient has insurance and Dr. Greenly participates with it, their office visit may be covered as a standard medical office visit and submitted with the customary co-pay billed to patient. Patients who have no insurance coverage will be charged $100.00 for the office visit, due at the time of service.
Per the State of Hawaii, renewals must be done on a yearly basis. Patients of Dr. Greenly’s can make appointments up to 60 days prior to expiration of their certificate and in most cases will not need to resubmit any medical documentation. Cost for renewals, unless otherwise updated, will remain the same.
